Isoretinoic acid belongs to the first generation of retinoic acid and is a stereoisomer of all trans retinoic acid. Oral administration has anti lipid effects and special therapeutic effects on severe acne. The mechanism of action is as follows:

① It has the function of shrinking sebaceous glands, inhibiting sebaceous gland activity, and reducing sebaceous gland secretion, thereby inhibiting the proliferation of Propionibacterium acnes that relies on lipid environment growth.

② It has anti keratinization, suppresses excessive proliferation of epidermal cells, and promotes their differentiation, which can reduce keratinization of hair follicle opening and sebaceous gland ducts.

③ It can also affect the function of monocytes and lymphocytes, inhibit the chemotaxis of neutrophils, and have anti-inflammatory activity.

④ Selective binding of retinoic acid nuclear receptors to exert therapeutic effects. Inhibiting the proliferation of Acne short rod bacteria has an immunostimulatory effect at low doses and an immunosuppressive effect at high doses.

⑤ It can inhibit the formation of collagenase and gelatinase in the skin, and inhibit the activity of ornithine decarboxylase. Since the induction of ornithine decarboxylase can play a major role in skin mutations, isotretinoin can also inhibit tumor development.

⑥ When applied topically, the mechanism of action of this product is similar to that of retinoic acid. Pharmacokinetics:

After oral administration, this product is rapidly absorbed and reaches the peak blood concentration within 2-4 hours. The oral bioavailability is low, and postprandial medication can increase bioavailability. The binding rate with plasma proteins is greater than 99%.

Mainly metabolized in the liver or intestinal wall, it enters the enterohepatic circulation in its original form and metabolites. Unchanged original form drugs are excreted from feces, while metabolites are excreted through urine.

The elimination half-life is 10-20 hours.

Indications
